Monroe House uses the WeCare system to determine which of the seven levels of assisted living care our community offers, will best fit the resident. When a family decides that they may want their loved one to come to Monroe House, a nurse goes out to meet the potential resident and does an evaluation. The evaluation is entered into the WeCare system and a individualized care plan and level of care is generated. The care plan will explain how the resident will be taken care of by our staff. The care plan includes all activities of daily living, medication management, meals, laundry, housekeeping and activities. Our resident care director, Akua will review the care plan with the potential resident and family to make sure everyone agrees with the plan before implementing it. When the resident moves into the community the care plan is reviewed again after 30 days . We offer transportaion (within a 15 mile radius) on Tuesday's and Thursday's to doctors appointments on our community bus.
