When a new resident joins our community, we create an individualized Service Plan, called We Care, which details the specific services the resident needs. They may need help with medications, bathing or dressing, assistance getting to and from the dining room – whatever help they need, we can provide. No matter what they need, our nurses have worked in the home health industry for many years and as a result, are extremely familiar with Medicare and know what Medicare will pay for and what it doesn’t cover. So as a resident’s care level rises, we thoroughly understand how to use the Medicare benefit to help cover those costs, reducing out-of-pocket costs for our residents.

Alzheimer's and Dementia Care Services

Join Their Journey is a wonderful program based on the idea that we join the resident wherever they are – whether it’s years ago, back in their own home, still raising young children. For example, one resident told us she was so glad she didn’t move out of her home in New Jersey. So we validated her by saying how happy we were that she stayed and put her New Jersey address on her life station outside of her doorway. We don’t argue or say no. The truth is what they are thinking – if they think their shirt is purple, but it’s really red, we will validate what they are saying, which helps alleviate anxiety and frustration for the resident.

Before we can join their journey, we need to learn as much as we can about the resident. Toward this end, family members complete a 20-page life story, documenting many details about his or her past life. We can then create an environment that reflects this life story to help promote memories. For example, we built a sand box for one resident who loves the beach. When she put her toes in the sand, she remembered that she had been in Hawaii for her honeymoon. Giving residents a chance to talk about their past helps them feel respected and helps them feel part of our community – and they are; they are the most important part of Emeritus at Sugar Land.
