In many cases, our Licensed Vocational Nurse will visit a potential new resident in his or her home, which helps our staff get a good feel for the resident’s health concerns and preferences. From this, we put together an assessment of their needs. This, combined with other medical information, is used to create an individualized care plan, which is reviewed by our nurses on a quarterly basis or sooner, if the resident’s condition changes. Our goal is always to help residents remain as independent for as long as possible, while making sure they are safe. And our staff is always in constant communication, so that if a resident is not doing well, we can quickly respond and either get a nurse from home health or doctor to come in for a visit. If a resident needs to be hospitalized, we keep close tabs on them, often visiting and taking them personal items and checking on their progress.
