Assisted Living Services
Before a resident moves into Heron Pointe, one of our nurses conducts a thorough assessment of his or her health status, from which a personalized care plan is created. To ensure that residents pay for just the services they need, we offer four levels of care. We can also add extended or end-of-life care by bringing in hospice as necessary.
Care plans include the full range of assisted living services required, with our staff always walking a fine balance of making sure residents are cared for, without taking over tasks that they can do for themselves. Encouraging them to do whatever they can for themselves is key to helping residents maintain a sense of independence and control.
We recognize that conditions can decline or improve, so taking the time to do a second assessment 30 days after move in helps ensure that we are meeting the resident’s exact needs.
