Alzheimer's and Memory Care

Some residents require higher levels of assistance due to Alzheimer’s, dementia or other memory loss disorders. For these seniors, we offer an innovative program called Join Their Journey®. Our goal is to get to know each resident and his or her family, so that we can offer the right support for every stage of the disease. Through this approach, we recognize individuality, encourage choices and help residents maintain their dignity by creating a safe, personalized and familiar setting that feels comfortable for them. By enabling our residents to enjoy daily events that are both familiar and pleasurable based on their Life Story, by providing a relationship-based care plan, and by using multi-sensory experiences such as our unique life stations, we are able to join their journey through the various stages of the disease.

To ensure the highest quality of care, our Resident Care Director creates a care plan specific to each resident. The care plan is created using a comprehensive evaluation tool that compiles information about what care assistance each resident needs; how often they need assistance and any preferences associated with their care or daily routine. This evaluation is completed regularly as well as when there is any notable change in needs or preferences. This way the care needs of our residents can be reviewed with our resident and families. Emeritus at Outlook Manor is locally managed and has the backing of a national company. Our comprehensive evaluation tool is just one example of the many resources we have available to us and our families.
