Before a resident moves into Heritage Hills, we conduct a full assessment that includes a personal history, which helps us get to know the individual before he or she became ill. This, along with other pertinent medical information, forms the basis of the care plan and covers whatever activities of daily living are required. The plan is reevaluated quarterly or if the resident’s condition changes. Our team takes great pride in caring for residents – if a resident came out of her apartment and needed her hair brushed, we would take care of their grooming needs. Helping them maintain their dignity and honor is all part of our care.
