At Emeritus, your comfort isn’t compromised by a need for increased care. We offer the unique option of receiving short-term rehabilitation care in the comfortable and homelike environment of our Skilled Nursing Facilities, also known as Transitional Care Communities. You’ll receive the same senior care services all residents enjoy: home-cooked meals, housekeeping, transportation, outstanding service, and more.
Short-Term Rehabilitation
Many of our guests stay with us following a hospital stay to recover from an illness, injury or surgery, and either return home immediately following rehabilitation or choose an extended guest stay with us through our VIP Program before returning home.
We work closely with your physician to develop an individual plan of care that meets your specific goals and personal preferences. Your multidisciplinary care team includes your physician; our Medical Director; Director of Nursing; licensed nurses; certified nurse aids; a team of physical, occupational and speech therapists; a social worker; lifestyle coordinator; and nutrition manager. Our short-term skilled nursing and rehabilitation services include:
- Post-hospital and post-surgical care
- Intravenous (IV) services
- Physical, occupational & speech therapy
- Cardiovascular care
- Orthopedic recovery
- Behavioral and cognitive care
- Wound management
- Coordinate Dialysis Care
- Incontinence, catheter and colostomy care
- Transition of care coordination
- Home visit program following discharge
Most of our skilled nursing communities are Medicare-certified and are able to accept Medicare and insurance reimbursement. For more information on financial eligibility, please call the community nearest to you.
Remain at the Same Community Even if Your Health Needs Improve
As your health conditions improve, you may decide that having a helping hand – perhaps someone to help manage medications, help get you ready for the day or prepare special meals – is beneficial to you or your loved ones’ situation. Through our VIP Extended Stay Program you may find that sometimes that extra help is all we need to continue living safely on our own.
Stay at the same continuing care retirement community without the stress of moving. Emeritus offers all the extra help you may need, but only when and if you need it. Our full range of independent living, assisted living, and memory care services are designed to help you maintain or improve your health, so you can live as independently as possible.
Long-Term Nursing Care
When circumstances require 24-hour care, you can trust Emeritus for your long-term nursing care. Together with your physician, our multidisciplinary team is dedicated to meeting your specific care needs through the development of an individualized care plan. Coordination of a wide variety of services, including restorative care, palliative care and hospice services, is available when needed. We encourage family engagement and offer opportunities for involvement through our life enrichment and dining programs, and through our care planning processes.
Whether you select Emeritus for your short-term or long-term nursing care, you will share the experience that our residents, families and staff have in knowing that we are true to our brand promise: Our Family is Committed to Yours.
