Assisted Living Services
Our residents are not able to tell us much about themselves, so we work closely with our family members to develop a care plan customized to the resident. This plan, which details as much information about their past life as possible, their likes and dislikes, medical history, routines and much more, helps our staff get to know each resident. The care plan can include as much or as little assistance as required – we do have some residents who are in the early stage of their disease, while others require us to do everything for them. As much as we can, we encourage our residents to do what they can for themselves, helping them maintain their independence. For example, we’ll provide queuing and reminders for them to brush their teeth, versus doing it for them – unless they require total assistance.
After 30 days, we reevaluate the care plan to be sure we are providing the appropriate level of assisted living services. Every 60 days or so, we have a care conference with our families to ensure they are kept up to date with the status of their loved one. We also have a Family Council that meets quarterly, providing a structured way for us to address any concerns or issues family members may have.
Alzheimer's and Dementia Care Services
As an Emeritus community, we follow Join Their Journey, a program that allows us to work closely with each individual resident and to join them in their journey, wherever they are in the disease process. If a resident were to say to us, “I want to go see my husband,” but her husband has passed on, we don’t remind them of that reality, but rather redirect the conversation. In her mind, her husband is still alive; that’s her reality and we become a part of that reality.
An important part of Join Their Journey is engaging residents one-on-one. Toward this end, we have memory stations set up throughout the community – a wedding station, where we can talk to them about when they got married; a sewing box with items that help them remember their love of sewing – to name just a few. We also have activities that allow them to be productive, such as putting dishes away, folding clothes and setting tables. These are activities they enjoyed before they became sick and help them maintain a sense of purpose.
