Assisted Living Services
Emeritus at Hazel Creek has assisted living services. The care plan is done specifically for each resident. Every one of our apartments is licensed for care. Many residents choose us for independent living because they don't want to move again if they need care. We partner with local home health agencies and physical therapy agencies as a resource for health needs.
Alzheimer's and Dementia Care Services
Our community also offers an innovative memory care program, Join Their Journey®, for seniors affected by Alzheimer's or dementia. Because your family's experience will be unlike any other, our goal is to get to know you and your loved one, so that we can offer support and solutions for your individual situation. We "join the journey" of your loved one by recognizing individuality, encouraging choices and allowing residents the dignity to live in a secure, personalized and familiar setting that feels comfortable for them.
Our senior living community is designed to care for seniors with Alzheimer's, Dementia and/or memory loss issues through various services and amenities. These services and amenities will be based on each resident’s needs.

- Choice of 505-sq.-foot one bedrooms, 446-sq.-foot junior one bedrooms and 640-sq.-foot two bedrooms/one bath

- All utilities except basic cable and phone included
- Pets welcome (up to 25 lbs.)
