Assisted Living Services

For each resident, we create an individualized care plan, which is reviewed on a regular basis (every 90 days for our memory care residents; every 6 months for our assisted living residents). We also work closely with the family members and the resident’s physician to be sure they feel comfortable with the plan. With all of the tasks clearly spelled out in the care plan, our caregivers know exactly what assisted living services the resident requires. Should they notice that the resident is having difficulty doing something – for example, asking to be pushed in a wheelchair, when they should be able to walk on their own – the staff member fills out a “care alert.” This lets our Resident Care Director know something is amiss, and she’ll talk to the resident to find out what’s going on. If necessary, we will bring in additional services to help the resident overcome whatever difficulty they may be having, with our ultimate goal always to help residents remain as independent and able to do for themselves as possible. In addition, we provide regular health and wellness checks, including various health screenings, as well as coordinate onsite physician visits as required. We also provide transportation to medical appointments.

Alzheimer's and Dementia Care Services

As part of Emeritus, we structure our memory care community around the Join Their Journey program. This innovative program has several components focusing on all aspect of our residents’ care, from proper nutrition to memory stimulation to life enrichment. The resident is at the center of the program – we always join them in their journey, never arguing with them, but being with them wherever they are at the moment. If a resident prefers to wear socks instead of shoes, that’s fine with us. Everything we do is designed around their needs – to make sure they are comfortable in their environment, to promote good brain health and socialization, and to ensure their safety and wellbeing. For example, we have specific activities that stimulate the senses – touching materials or bringing in familiar scents – to help them recall memories from long ago. Residents typically are living in the past – sometimes 10 years ago, often much longer. So when a resident first moves in, we work closely with the family to uncover their life story. From this, we create “shadow boxes” filled with photos of loved ones and other reminders that help create the past for them. In addition, we have set up “memory stations” – areas in the community that are furnished with record collections from the 50s and 60s, baby dolls and changing tables, a dresser with clothes and accessories from years ago and other familiar elements that make them feel comfortable and at home. We also take our residents, many of whom are from the immediate area, on country rides around Tracy to help them recall memories from their old neighborhoods. Our community also offers a secured courtyard, so that residents can go in and out as they like, but within a safe environment that is continually monitored by our staff.
