Assisted Living in Stockton, CA

Assisted Living Services

When a new resident first moves in, we do conduct an initial evaluation to determine the level of services required, and we. We reassess 30 days later after a resident physically moves in to ensure that we are meeting their care needs and the family’s needsexpectations as well as adjust the care plan accordingly. Each resident is provided a designated caregiver – one Resident Assistant each per shift – who provides the services needed. The individualized care plan can include any combination of daily assistance with bathing and dressing, dining assistance and special diets, assistance with reminders and redirection, escorts and assistance with walking, specialized therapy services and/or hospice care. We always encourage residents to do as much for themselves as possible, so that they can remain independent and active for as long as they are able. Residents are easily able to stay in our community and “age in place” by adding services as needed. They can also move into our memory care community, which is located in the same building, should they require these specialized services. To make the transition easy, and depending on the situation, the resident can continue living to live in his or her assisted living apartment and simply participate in the daily memory care activities until they are ready to make the full transition.

Alzheimer's and Dementia Care Services

Our Memory Care Neighborhood has 16 studio apartments, each of which can be turned converted into a Friendship Suite – an ideal option for residents in needthat enjoy of companionship. In keeping with the Join Their Journey Program, which is focused on our joining residents in their reality – not them joining ours – we have individual “life stations” set up throughout our community. These include a businesses station for the men in our community who were once business executives, a church station for our ladies who were active in their churches, a sports station and military life stations for our veterans. These life stations allow our residents to reminisce, enjoy and talk about activities that are meaningful to them. Whatever they are focused on – whatever they tell share with us – we never disagree, but we join and engage them in the conversation or activitywherever they are.. For socialization, we offer several common areas, including a dining room that doubles as an activities area, living room with television and French doors leading out to a large secured courtyard with benches for seating and red brick pathway for walking. The courtyard is fully landscaped with different kinds of edible flowers and vegetables.
