the elderly affect the whole family, we’ve designed our Join Their Journey® program

with the support you need – all in a secure and inviting setting. All residents
in our Alzheimer’s and dementia care facility receive:

- Three-plus-three
meal and snack program specifically designed for Alzheimer’s and senile dementia
residents
- A warm friendly and safe environment with a secured outdoor area
- Personalized activities plans
- Scheduled transportation
Plus, we individualize our Service and
Care Plan to help with reminders, redirection, medication, bathing and other specific
needs. Services can include:
- Assistance with Alzheimer’s medications
- Dining assistance
- Personal laundry
- Escorts and assistance with walking
- Diabetic management*
* As allowed by state regulations.
The Join Their Journey Experience
We train our caregivers to understand how Alzheimer’s disease and dementia in the elderly affects the brain.
With this knowledge, our staff joins residents in their moment, rather than trying
to bring them into our reality. This is our Join Their Journey program,
based on the following philosophy of care:
- We create a familiar environment
that wraps residents in the comfort of past meaningful experiences by personalizing
their apartment and surrounding
common areas with items and “memory stations” that rekindle memories. Memory stations could include a table
strewn with every day tools, a teacher's chalkboard, a 1940s
vintage kitchen stocked with dishes and products from that
era, or an old-fashioned vanity stocked with scarves,
handbags, hats and jewelry.
- Our staff gets to know residents in our Alzheimer’s and dementia care
program personally by spending time
with them and their loved ones to build a personal biography. Then we create a Service and Care Plan based on the biography
details to ensure each resident feels comfortable, safe,
valued and well cared for.
- Plus:
You’re able to provide insights into your loved one’s preferences,
life stories, traditions, values and beliefs. So we keep you closely involved in care planning
and offer support groups and classes just for you and other family members.
Three-plus-three meal and snack program
Studies have shown that people with dementia or Alzheimer’s
eat better when food is offered several times during the day. So to accommodate
the special dietary needs of residents with Alzheimer’s disease or related dementia,
our communities offer the “Three-plus-three” meal and snack program offering:
- Three meals and three mini meals daily
- Nutrient rich menu items and beverages
available all day as desired and/or needed
- Improved nutrition to help residents
be healthier and have a greater sense of well-being
- More opportunities to interact
with other residents with Alzheimer’s or senile dementia
